Entry Checklist

1. Complete entry form no.1 with payment of $25 ( for up to two garments and $10 per each additional garment )by April 30th 2010. Paid by this date and you will receive one ticket for the show on 17th July 2010.1-3 pm  Century Theatre, Napier.

2. Pay entry fee $25 ( by cheque or direct credit to ASB Bank - A/C 12-3110-0007354-000 name reference to be included.)
    Make cheque out to – Annette Oughton / Deco Inspiring.

3. Arrange insurance of your garment.

  Garment Delivery.

4. Attach a copy of entry form no.2  from the website to your garment when sending or delivering. (with Name ,Address, Contact details) you can download from website.

5. Make sure you have the fabric label stitched inside your garment for identification.

6. Include the colour photo of your garment the way it is to be worn.

7. List the total number of pieces included with your delivery.

8. Garment(s) to be delivered before or by 30th June 2010.
